I am not able to secure the form roller. The screws at both ends loosen and it moves. As it does a small portion is chewed up in the gear track. How do I fix that? I have a video, but I am not sure how to send it to you.

I don’t think a video is necessary. I believe the roller core end screws are not the issue. The cores are moving laterally because of a loose or missing shaft collar.
The shaft collar should be set approximately 0.010″ from the Nyliner on the bearing block. NA Graphics should have them in stock, but I’ve included the spec to find it locally.
Additionally, the rubber on the core may have been built slightly too far to the right. A band saw or ant serrated blade can remove the excess rubber.
